Intake Process

Submit the Rising Adults Inquiry Form on this page.
Once the form is received, our Program Director will reach out to schedule a tour of our homebase during day programming. Before your visit, please take some time to review the policy & disclaimer linked below.
At the end of the tour, you will receive access to complete our full Program Application & Waivers to complete prior to eligibility for program enrollment.
Upon application approval and based program availability, eligible participants will be able to enroll and pay for our Day Program(s) and/or Social Club events.
