RISING ADULT PROGRAM INTAKE

Once the form is received, schedule a tour of our facility using the link in the form submission confirmation email.
- Tour location: 2015 Dean Street (Unit 5), St Charles, IL 60174. Please see the map below as well.
- Before your visit, please take some time to review these documents: New Family Trial Period & Enrollment Disclaimer and Aggression Policy.
3. At the end of the tour, you will receive access to our full Program Application & Waivers to complete prior to eligibility for program enrollment.
- *Note - All new families are required to submit two letters of recommendation prior to or during the trial period. Letters must be from individuals familiar with the participant in a structured setting, such as a previous school, a prior program or service provider, a member of the participant’s IEP team.
Upon application approval and based program availability, eligible participants will be able to select and purchase monthly Day Program passes and/or sign up for Social Club events.
- *Note - New participants will be subject to a minimum two-session trial period before formal enrollment. More information is provided during program tours.
